This form needs to be completed to create online registration for an event.  With the increasing number of online registrations the council is managing, a system has been implemented to track each request. This system helps the council not only do a better job with posting information, but also provides information required for accounting and auditing.   Please complete the fillable PDF file and have your District Executive or council manager forward it to the Council Business Manager.  The form tells the council the information that will collected via the online registration page for the event.
